A sperm analysis, also called a semen analysis, examines a semen sample in a laboratory. It commonly measures semen volume, sperm concentration, total sperm number, movement and shape. The test is private and usually straightforward, but correct collection, prompt delivery and laboratory quality are important for a meaningful result.

Before the test

Follow the clinic’s collection instructions. You will usually be asked to avoid ejaculation for a specified number of days. Tell the clinic about fever, recent illness, medicines, supplements and any collection difficulty because these can affect interpretation.

Do not use ordinary lubricants, saliva or a standard condom unless the laboratory specifically provides an approved collection method. Some products can interfere with sperm movement.

How the sample is collected

The sample is usually produced by masturbation into a clean, sterile, labelled container in a private collection room or, where the laboratory permits, at home. The entire ejaculate should be collected because the first portion often contains many sperm.

If any part is missed, tell the laboratory rather than trying to conceal it. Home samples must be delivered within the time and temperature conditions given by the clinic.

What happens in the laboratory

The sample is allowed to liquefy and is checked using standardised laboratory methods. Assessment commonly includes:

  1. Volume and physical characteristics of the semen
  2. Sperm concentration and total sperm number
  3. Motility: how many sperm move and how they move
  4. Morphology: the proportion with a typical shape
  5. Vitality when needed: whether sperm are alive
  6. Other observations, such as pH or cells that may suggest inflammation, when clinically relevant

How results are interpreted

A semen analysis is not a simple pass-or-fail fertility test. WHO reference distributions do not create a strict border between fertile and infertile men. Pregnancy depends on several semen measures and on the reproductive health of both partners.

A mildly abnormal result may be repeated because semen changes over time and can be affected by fever, incomplete collection, abstinence period and normal biological variation.

What happens after an abnormal result

The fertility specialist may recommend a repeat sample, examination, hormone tests, ultrasound or genetic evaluation depending on the pattern and severity. Do not start testosterone or fertility supplements without medical advice.
